If you got your April 2026 paycheck and thought "wait, why is the health insurance bigger?" — you are not alone. From April 2026, every health insurance subscriber in Japan pays a new surcharge called the Child and Child-rearing Support Contribution (子ども・子育て支援金).
It does not matter if you have children or not. It does not matter if you are Japanese or foreign. Everyone pays.

1. What Is the Child & Childcare Support Levy?
The Child and Child-rearing Support Contribution (子ども・子育て支援金, kodomo kosodate shienkin) is a new fee added to your monthly health insurance premium starting April 2026.
The Japanese government wants to fund:
- Bigger child allowance (児童手当) for all families
- Free standard childbirth (no co-pay for normal delivery)
- "Childcare for All" daycare program
- Maternity benefits
Total funding goal:
- FY2026: about ¥600 billion
- FY2027: about ¥800 billion
- FY2028: about ¥1 trillion (full implementation)

2. Who Pays?
Everyone who has any kind of Japanese public health insurance:
- Company employees (健康保険組合 or 協会けんぽ)
- National Health Insurance (国民健康保険, 国保) - self-employed, students, freelancers
- Late-stage elderly system (75+ years)
That is about 100 million people in Japan.
Foreigners: YES, you also pay. If you have a residence card and any health insurance, you pay. No exceptions for foreigners.
3. How Much Will I Pay? (Full Salary Table)
The fee depends on your salary and insurance type. Here are the official estimates from the Children and Families Agency.
For Company Employees (Shaho 社保):

Important: Your company pays half. So if your salary is ¥6 million, YOU pay ¥575 and your company pays ¥575. The ¥575 is YOUR side only.
For National Health Insurance (Kokuho 国保):
This is for self-employed, freelancers, students, and unemployed. You pay 100% yourself.

4. Why "Singles Tax"?
On Japanese social media (Twitter/X), this fee is being called "singles tax (独身税)."
Why? Because:
- People with children get bigger child allowances (児童手当)
- People without children just pay, get nothing back
- Singles, childless couples, gay couples, and people in their 20s/30s feel they pay for others
This is unfair to many. But the government's view is: "Japan's birthrate is collapsing. Everyone benefits from a future generation of workers."
Yamada honest opinion: Both sides have a point. As a parent in Japan, I appreciate the support. But I also see why my single friends are angry. ¥575/month is small for one person, but ¥6,900/year × 100 million people = serious money.
5. Where Does My Money Go?
The new revenue funds 6 specific programs (set by law):
If you have a child in Japan now, you DO benefit. If you do not, you pay for the system.
6. National Health Insurance (国保) vs Company Insurance (社保)
If you work for a Japanese company:
- You have Shakai Hoken (社保) / Kenpo Kumiai (健保組合)
- Surcharge is deducted from your paycheck
- Company pays half
- You will see a new line on your paystub from April 2026
If you are self-employed, freelancer, or unemployed:
- You have Kokumin Kenko Hoken (国保)
- Surcharge is added to your annual bill
- You pay 100%
- Bill comes from your city office
Student visa holders:
- Most students are on Kokuho (国保)
- The surcharge applies
- It is small (~¥150-¥300/month if income is low)
7. How to Read Your Paystub
Look at your April 2026 paystub. You should see a new line.
Possible names (in Japanese):
- 子ども・子育て支援金 (Kodomo Kosodate Shienkin)
- 子育て支援金
- 支援金
- Often included inside 健康保険料 (health insurance) as a sub-line
If you do not see it explicitly, look at your total health insurance amount. Compare to March 2026. The April number will be slightly higher.

You cannot avoid the surcharge legally. But you can save money elsewhere.
Tip 1: Use Furusato Nozei (ふるさと納税).

Tax-free growth. Long-term wealth offsets the small monthly fee.
Tip 3: Use the new childcare benefits IF you have kids.
- ¥30,000/month from 3rd child
- Free standard childbirth from FY2026
- Free hourly daycare (¥300/hour for parents not working)
Tip 4: For freelancers - consider switching to Shaho.

9. Real Stories
Carlos (Brazil, Engineer, Salary ¥5,000,000):
Carlos pays ~¥479/month new surcharge. Per year: ¥5,748. He has 2 kids, so he gets ¥30,000/month child allowance. Net benefit: positive.
Sophie (France, Single, English Teacher, Salary ¥4,000,000):
Sophie pays ~¥383/month. Per year: ¥4,596. She has no kids and gets nothing back. She feels the "singles tax" anger but says "okay, it's ¥4,600/year, not a disaster."
Rina (Indonesia, Caregiver, Salary ¥2,500,000):
Rina pays ~¥239/month. Small amount. She sends remittance to family in Surabaya. The new fee reduces her remittance by ¥2,880/year. Not big, but she feels it.
Wei (China, Student, National Health):

Q1: I am a foreigner, do I have to pay?
Yes, if you have any Japanese health insurance.
Q2: Can I opt out?
No. Like all health insurance fees in Japan, it is automatic.
Q3: My company hides this in "social insurance." How do I know?
Compare your March 2026 paystub to April 2026. The difference (after tax adjustments) is mostly the new surcharge.
Q4: Will the fee keep going up?
Yes. 2026 is the start. By 2028, it will be roughly 1.7x higher.
Q5: I am leaving Japan in 2027. Do I pay?
Yes, until your last day. Then your insurance stops.
Q6: Will my pension refund (脱退一時金) be reduced?
No. Pension refund is separate from health insurance.
Q7: Does this affect my income tax?
Health insurance premiums (including this surcharge) are deductible from your annual income tax. So your total tax is slightly lower. Small consolation.
Q8: My spouse has no income. Do they pay?
If they are your registered dependent, no. They are covered by your insurance.
Q9: What if I get fired or quit?
You will switch from Shaho to Kokuho. Both have the new surcharge.
